Remote care is not simply a device deployment. It is a care model that requires defined eligibility, patient onboarding, monitoring responsibilities, escalation pathways and service evaluation.
Define the population
Specify which patients may benefit, which conditions or risk signals are monitored and what would make the service inappropriate.
Design escalation
Every alert should have an owner, response expectation and documented action. Avoid creating large volumes of low-value notifications that overwhelm clinical teams.
Support participation
Consider language, connectivity, device access, caregiver needs and digital confidence. Provide alternatives for patients who cannot use the standard pathway.
Measure meaningful outcomes
Track clinical outcomes, utilization, adherence, experience, equity, staff workload and total cost. Technology metrics alone cannot establish program value.
